These notes are intended for minor, non-emergency conditions and do not replace an in-person physical exam when required by an employer, school, sports program, or travel authority. Arc Med Health reserves the right to decline issuing documentation if a patient’s condition requires in-person assessment or does not meet clinical criteria.
